If a child has to take medication prescribed by a doctor or dentist during the school day, parents should notify the school. A MED 1 form must be completed to accompany the medication. The MED 1 form allows school staff to administer medication to the child. 

Please contact the school office if you require a printed copy of the form.

Prescribed medicines should have a pharmacy label and be clearly marked with your child's name, the date prescribed, how often it has to be taken and for how long.
